I bill for a mobile radiology company. We go to the patients’ residences, primarily nursing homes - Place of Service (POS) 32. Until January 2015 we billed and got paid for ultrasound of the breast, CPT 76645. That code has now been replaced with 76641 & 76642. Our Medicare Contractor - Novitas Solutions has been denying these claims with the Reason Code Co-58 – Invalid Place of Service

I submitted an appeal and received a negative response. I cannot find anything directly from Medicare or Novitas that there has been a revision on the acceptable POS for this service.
Is anyone billing for POS 32?
