Pt. with history of migraines presents to MD with typical migraine symptoms sensitivity to sound and light, throbbing headache on left side of the head, and nausea. Vital signs normal. No history of hypertension. Pt. was given intramuscular injection of Demerol and Phenergan and sent home. When pt. rec'd bill, it was coded as 401.1 Hypertension benign, 558.9 Colitis, 346.10 Migraine without status migrainous. Pt. called office and questioned these diagnoses. She has no history of hypertension or colitis. Should we consult with MD and get correct codes and resubmit statement to insurance and would this make the pt. have a permanent diagnosis of hypertension and colitis and if so how would we and the pt. correct this. (Sorry but the question mark and forward slash key on my keyboard won't work.) The pt.'s office visit was coded as 99214. Thank you.
