Q&A Occurrence codes

I would like to learn more about occurrence codes. I code ancillaries, referred and ER dx at a hospital, but we had a crash course in CCI edits. What I mean by crash course is that they gave us about a one hour lecture and then let us loose with CollectLogix.

The business office sends us "issues" to correct like CCI edits and occurrence code mismatches -among others.

My question is about occurrence codes. It seems that the more I ask, the more confused I get. Ever feel like you get a different answer every time you as a question???? haha I even came home and searched my textbooks for more guidance on occurrence codes. No luck there.

Pt comes in after a falling down some steps. Occurrence code is 05. What if the patient comes in later in the month for continued pain? Is the 05 still appropriate? It's not a new accident, but the pain is the result of an accident. Or would it be 11?
I found out today that our admission/registration folks apply the occurrence code based on how the pt answers the question "is this the result of an accident"? By that standard, a pregnancy could require
an 05! haha Basically we are correcting occurrence code/ICD9 code mismatches, so I need a better understanding of how to determine which occurrence code is correct. Thanks
