ICD Hx of Cancer Query

The Doctor Sts the cancer is gone. Can this be considered Hx of cancer, or does he need to use that exact phrase, Hx of cancer?

I'm pretty sure you can use the V code, but I want to make 100% sure.

An Addendum: Pt had treatment for this. A future document from her PCP sts "cancer is gone". He addresses other issues that have arisen, from after Tx, but that are considered separate issues.

As I read further, there is a recommendation at the time of encounter with the PCP that sts "No one has recommended chemotherapy as far as adjuvant therapy at this time, and we will just basically follow her along clinically."

I think I answered my question, lol. It's not a Hx code.

Yes you did Chaim. I was going to tell you yes but to look into to it and confirm she is not taking tamifan or the like for breast Ca. Excellent research.
