ICD First listed Dx issue

According to the Icd 9 manual, code v72.85 should not be listed first.

However, it is the only code being listed on an outpatient superbill for a medical clearance.

Another code/s is/are needed prior to v72.85, correct? Or is there a guideline that I'm not paying attention to which will allow this?

In the tabular, I don't see any guideline directing me to NOT use it as a first listed code. Where are you seeing that rule?
