I work for a family practice (patients 18 yrs +); we see a lot of patients with diabetes (with and without manifestations), Alzheimer's, Palliative Care, CKD and CAD just to name a few ... I have over 6,000 patients in office. We also go to 10 nursing home / assisted living facilities, 2 hospice centers and 2 hospitals.
