Patient came in for a yearly routine physical exam and has chest pain a night before he/she came in. Can the doctor bill both preventive (ex. 99326) and office visit (ex. 99213) with modifier 25?

Yes if they have a truly separate and identifiable regular EM visit. It's even advised to have two separate notes. And yes you'd use a modifier 25 on the regular EM visit (not the PM visit).
