This was covered briefly in the October 2013 Q&A Webinar by Alicia Scott, CPC, CPC-I. The GEMs are data files and list the ICD-9 and ICD-10 codes and the attributes of the mapping between the 2 code sets. There are GEMs that map ICD-9 to ICD-10 called "forward mapping" and GEMs that map ICD-10 back to ICD-9 for "backward mapping". The GEMs are available for free and downloaded from the NCHS website,
