Per cpt code description, this concurrent infusion code can be used one time per encounter (defined per Medicare). In the cpt assistant it seems to indicate per day. I have seen a lot of denials from payers when this is used per day....reasoning is that per cpt, this is a per encounter code. what is correct?

Never mind on this. I just noticed that same issue was addressed in the 2012 cpt code changes (changed from "per encounter" in 2011 to "per day" in 2012...duh!
By the way, I did not give my accurate birth year when I signed up for this forum...did not feel that was necessary for us old gals!
