Break down a medical report to......

Follow up to an email I sent to Laureen yesterday........The break down of a report was never covered in depth in my coding course (this is a home correspondence course). It was just the basics and some refresher on things I have studied before (such as HIPPA, etc). In my last two exams I had in depth examples of clinical notes, operative report, just to name a few which I coded to the best of my ability but when I got the exam back I did okay. All I received was a list of reference numbers to review and not a full detail on explaining why I was incorrect. They even marked another answer wrong which was actually right (I stood ground and told them to give me my credit back - they did). Another matter in my book is some discussion on ICD-10 but I should not study or be concern at this time. Later on I will send the a question or two on what I had on the exam and if I may receive comments on what I did incorrectly. I would be most appreciative.

Question: Is there a manual out there with answers to examples of actual reports (edited for educational purposes) that I may purchase to know how to break down and properly code whether its one code or multiple codes and the proper sequence? Thanking you in advance.

The Official 2013 CPC® Certification Study Guide will be available from aapc.com Jan.2, 2013. The earlier version is sold out. I have a previous version which I purchased for about $70. Each chapter that discusses CPT codes (chapter 5 - 18) has a couple operative reports to code with answers and rationales in the back of the book. There are also less complicated practice questions.

I, personally, do not own a book that is devoted to operative reports but there are some on Amazon. Search for: medical coding operative reports. I can't recommend them because I have not used any of these, but they exist.
