An AAPC representative recently emailed me this information:
Please make a note that when you are billing two CPT codes then we will use modifier 59 only with one CPT. Modifier 59 will be used with the CPT which has less billed amount because we will received 50% of the allowed amount once we will apply the 59 modifier.
---> I do not do billing at my workplace, so this type of information is all new to me. I am not really sure where I can find a book that contains this type of information. I guess it is something that we learn by experience (maybe different payers have different policies on this).
