This mini medical record was listed in a book of exercises I'm working on for ICD-10.
Chief Complaint: Patient states she ran out of her anticoagulant Warfarin 5 days ago and now "feels funny".
Assessment: Lupus Anticoagulant Syndrome, Systemic Lupus Erythematosus.
Plan: restart Warfarin, FU in office
My question is would the fact that the patient ran out of her anticoagulant and presented to the hospital not feeling well be considered an adverse effect?
